We are looking for a highly motivated Practice Clinical Pharmacist with General Practice experience to join a friendly, dynamic GP practice in Reading.
You will work closely with our practice teams to deliver our pharmacy vision, providing excellent pharmacy‑led services and driving high‑quality, safe and cost‑effective prescribing for our registered patients.
You will be joining a supportive team of 7 Partners from diverse backgrounds, caring for approximately 14,000 patients in an EMIS Web practice with a full range of enhanced services. We encourage daily morning coffee breaks at 11am to catch up with colleagues.
We are most interested in candidates with
- Up‑to‑date clinical knowledge and a thorough understanding of primary care prescribing and medication reviews, including experience of designing and implementing strategies for improving prescribing.

- The ability to recognise your own professional limitations and boundaries, understand those of colleagues, and refer appropriately when needed.

Key responsibilities
You will work as part of a multidisciplinary team to:
- Deal with the repeat prescription system and acute prescription requests, and manage medicines on transfer of care, including discharge letters, treatment advice notes and medicines reconciliation.
- Provide telephone clinics and remote/face‑to‑face consultations for patients with questions, queries and concerns about their medicines.
- Perform structured medication reviews and support delivery of aspects of the Quality and Outcomes Framework (QOF) and enhanced services.
- Work with the practice to develop and apply systems for safer, more cost‑effective prescribing.
- Develop and maintain good relationships with local community pharmacies and the PCN pharmacy team.
- Take part in periodic meetings to identify and review prescribing and patient safety issues.
- Contribute to the Prescribing Quality Scheme, PINCER and QOF‑related prescribing audits.
- Work with care home staff to improve safety of medicines ordering and administration, including periodic care home visits.
- Work efficiently with other members of the pharmacy team, clinicians and the admin team.
This role is pivotal to improving the quality of care and operational efficiency within the practice and therefore requires motivation and passion to deliver excellent service within general practice.

- Experience working as a pharmacist in General Practice.
- Experience of clinical pharmacy and medication reviews, ideally in a primary care setting.
Desirable
- Experience of leading or contributing to prescribing audits, safety initiatives or service improvement projects.
Qualifications
Essential
- Degree in Pharmacy and registration with the General Pharmaceutical Council (GPhC).
Desirable
- Independent Prescribing Qualification (or working towards this).
